By going to court and winning their claim, the contractor Multiplex lost £2m – a cost that ultimately was blamed for the sale of this previously successful 43-year-old company. At first glance, this might not seem like too much of a failure – with a cost increase of just 32% (in part down to procurement route). But the cost in legal bills was phenomenal – Multiplex’s costs on a sub-trial alone (dealing with only a few of the issues at dispute) were quoted as being £45m.

UK newspaper The Telegraph commented in 2007: “The Wembley hangover has dogged Multiplex and seen the company struggling to restore its reputation just three years after it was listed in Australia.”

SCENARIO

Catherine is a 20-year old homeless woman, who is well known to the emergency department (ED).  Catherine has been admitted to the ED following the development of an abscess on her left side gluteus maximus. She states, to Nancy the RN, that she developed the abscess due to staying at a friend’s house for one night a few weeks ago. Nancy has seen Catherine on numerous occasions in the ED and is aware of her regular IV drug and alcohol use and feels that the injury may be as a result of injecting drugs into the area. During this visit Catherine has mentioned to Nancy that she maybe pregnant. Catherine currently lives in a women’s shelter and is concerned as she doesn’t want to bring a baby up within a shelter environment. Nancy discusses her concerns regarding the uncertainty of Catherine’s sexually transmitted disease (STD) status and the possible need for surgical intervention, with a senior colleague Sammy. Sammy agrees that Catherine may need surgical intervention and contacts the surgical registrar for review. Sammy also feels that while waiting for surgical review urgent bloods should be sent away for STD testing and pregnancy screening. After looking after Catherine for a few hours Nancy feels that social services may also be needed to help support her whilst receiving treatment and after discharge. Nancy askes Catherine her thoughts on receiving support from social services to which she responds “it depends if they treat me like real person or not. I mean there is more to me than being a druggie”. Nancy goes on tea break and is covered by a colleague Ben. On returning from tea she realises that Ben, during the administration of IM pain relief, has given himself a needle stick injury. Ben was not wearing gloves during the procedure, as per hospital policy, and is currently away from the ward having bloods taken and receiving counselling for the incident. The surgical registrar reviews Catherine’s abscess and feels that it can be drained using local anaesthetic. Sammy discusses the process, precautions and policies that will be need to be adhered to during the procedure with Nancy to prevent the potential transmission of STDs,  including HIV to other healthcare workers.

The case study continued

The blood test results for Catherine have returned from pathology. It was found that Catherine is Hepatitis C positive as a result of being an IV drug user. Nancy approaches Catherine along with the surgical register who informs her of the blood test results. Catherine is visibly upset by the news stating that she was “scared to get tested in the past and was anxious about the possibility of being Hep C positive”. The nurse stays and comforts Catherine and during this time thinks about some of the barriers and facilitators of Hepatitis C screening for IV drug users. Nancy decides at the end of her shift to go to the hospital library and carry out a search of the literature to identify some of the potential barriers and facilities to this.
